    Efficacy of 0.01% Atropine Eye Drops in Controlling Myopia Progression and Axial Elongation in Children: A Meta-analysis Based on Randomized Controlled Trials

    To clarify the preventive effects of 0.01% atropine eye drops against myopia progression and axial elongation in children, a meta-analysis was carried out based on data obtained from PubMed and Web of Science as of August 1, 2021. Randomized controlled trials (RCTs) that enrolled myopic children who had received atropine for at least one year were included in this study, Key search terms included myopia, children, and 0.01% or low-dose atropine. Heterogeneity was quantified by I2 statistics, and meta-analyses were performed using the fixed-effect model. Five RCTs involving 809 unique children were analyzed. One trial was excluded because of a poor Jadad score and markedly rapid myopia progression in controls. The mean effect sizes for 12 months in myopia progression and axial elongation synthesized from the remaining 4 RCTs were 0.20 (95% CI: 0.13 to 0.27) D and −0.08 (−0.11 to −0.04) mm, respectively (p<0.0001). The corresponding inhibition ratios were 28% and 19%. I2 statistics were 6% or less. Sensitivity analysis and funnel plots demonstrated the robustness of the estimation. The 0.01% atropine-induced inhibition ratio for myopia progression in Asian children was roughly half of that originally reported and did not reach the minimum requirement for clinical treatment

    Visual symptoms and compliance with spectacle wear in myopic children: double-masked comparison between progressive addition lenses and single vision lenses.

    The aim of this study is to clarify visual symptoms and compliance with spectacle wear in children kusing progressive addition lenses (PALs). Ninety-two children, participating in a randomized, doublemasked, crossover trial to determine whether PALs reduce myopia progression (mean+/-SD age: 11.0+/-1.6 years; refractive errors: 3.11+/-1.34 D), wore PALs (1.50 D near addition) or single vision lenses (SVLs) for 18 months, alternately. A questionnaire survey was performed 6 and 12 months after the beginning of the use of the lenses (6-month survey), and the results were compared between PAL- and SVL-wearing periods. In the PAL-wearing period, the children reported difficulty in adapting to newly provided spectacles (36%), disturbances in distance vision (22%), vertigo in the lateral gaze (11%), and difficulty in ascending and descending stairs (9%). However, the frequency of these symptoms was not significantly different from that reported in the SVL-wearing period. There was no difference in compliance with spectacle wear between the PAL- and SVL-wearing periods, and 98% of the children wearing PALs reported excellent compliance. The results of this study indicate that, compared with SVLs, the PALs provide a similar level of comfort and compliance with spectacle wear for myopic children.</p

    Genome-wide search for strabismus susceptibility loci.

    The purpose of this study was to search for chromosomal susceptibility loci for comitant strabismus. Genomic DNA was isolated from 10mL blood taken from each member of 30 nuclear families in which 2 or more siblings are affected by either esotropia or exotropia. A genome-wide search was performed with amplification by polymerase chain reaction of 400 markers in microsatellite regions with approximately 10 cM resolution. For each locus, non-parametric affected sib-pair analysis and non-parametric linkage analysis for multiple pedigrees (Genehunter software, http://linkage.rockefeller.edu/soft/) were used to calculate multipoint lod scores and non-parametric linkage (NPL) scores, respectively. In sib-pair analysis, lod scores showed basically flat lines with several peaks of 0.25 on all chromosomes. In non-parametric linkage analysis for multiple pedigrees, NPL scores showed one peak as high as 1.34 on chromosomes 1, 2, 4, 7, 10, 15, and 16, while 2 such peaks were found on chromosomes 3, 9, 11, 12, 18, and 20. Non-parametric linkage analysis for multiple pedigrees of 30 families with comitant strabismus suggested a number of chromosomal susceptibility loci. Our ongoing study involving a larger number of families will refine the accuracy of statistical analysis to pinpoint susceptibility loci for comitant strabismus.&#60;/P&#62;</p

    Clinical correlations of aggrecan in the resected medial rectus muscle of patients with intermittent exotropia.

    The purpose of this study was to elucidate the role of extracellular matrix components such as aggrecan, fibronectin, and laminin in the extraocular muscle of patients with strabismus. Resected tissues of the medial rectus muscle of 47 patients with intermittent exotropia obtained during recession-resection surgery were frozen under liquid nitrogen and pulverized by a Freezer/Mill to solubilize the tissue for enzyme immunoassay. The total amounts of aggrecan, fibronectin, and laminin in the resected tissue were correlated with clinical data of patients such as age, exodeviation, and refractive error. The amount of aggrecan decreased significantly with the advance of age (P &#60; 0.0001, Spearman rank correlation test), while the amount of laminin or fibronectin had no correlation with age. Patients with basic type intermittent exotropia showed larger, although not significantly, amounts of aggrecan than those with convergence insufficiency type (P = 0.0538, Mann-Whitney U-test). The amount of aggrecan may be related to motor aspects of intermittent exotropia.</p

    Time Trade-off Utility Analysis for Surgical Intervention in Comitant Strabismus, Glaucoma, and Cataract

    The utility value was compared among 3 surgical interventions, and the validity of the time trade-off (TTO) method was evaluated by analyzing the correlations of the utility value with the results of the Visual Function Questionnaire-14 (VF-14) and other variables. The subjects were 127 patients aged 40-85 years who were surgically treated between January 2008 and March 2010, including 26 patients with glaucoma, 50 with cataracts, and 51 with comitant strabismus. The scores on VF-14 and utility values determined using TTO were calculated retrospectively. The mean value (SD) of the utility gain was 0.096 (0.105) for glaucoma, 0.101 (0.105) for comitant strabismus, and 0.167 (0.237) for unilateral and 0.245 (0.167) for bilateral cataracts, indicating significant postoperative improvements in the utility value. A significant correlation was observed between the utility value and the postoperative VF-14 scores of the bilateral cataracts, and the postoperative visual acuity of the better eye of the unilateral cataract. The mean value of the quality-adjusted life years was 2.181 for bilateral and 1.424 for unilateral cataracts, 1.132 for strabismus, and 0.870 for glaucoma with an annual discount rate of 3%. The gain of utility value was highest in bilateral cataracts, and lowest in glaucoma, and thus the TTO analysis was considered to be highly valid for cataract surgery

    Body sway increases immediately after strabismus surgery.

    The purposes of this study were to examine whether body sway is altered immediately after strabismus surgery in children and to find preoperative clinical factors associated with body sway. In a prospective study, body sway was measured on 1-3 days before surgery and on the third day after surgery; for the measurements, computerized static stabilometry was carried out on 28 consecutive patients with strabismus (age range: 3 to 12 years old; mean: 7.4) who underwent strabismus surgery under general anesthesia. The linear length of the sway path (cm), the linear length of the sway path in a particular unit of time (cm/second), and the area of the sway path (cm2), indicative of the extent of body sway, all increased significantly among a total of 28 patients in both conditions of the patient's eyes open and closed, as well as among those in a subgroup of 16 patients with exotropia, after they had undergone strabismus surgery (p &#60; 0.05, Wilcoxon signed ranks test). The center of pressure along the Y axis of orientation from the toe to the heel was found to deviate significantly toward the heel postoperatively, as compared with the preoperative center in the subgroup of 16 patients with exotropia (p &#60; 0.05). Before surgery, 15 patients with no stereoacuity exhibited a greater amount of body sway when their eyes were open than did 13 patients with measurable stereoacuity (p &#60; 0.05, Mann-Whitney U-test). In the subgroup of 16 patients with exotropia when their eyes open, 3 patients with abnormal head posture exhibited more extensive body sway than did 13 patients without abnormal head posture (p &#60; 0.05). Body sway was found to significantly increase immediately after strabismus surgery in children with strabismus. Stereoacuity and abnormal head posture are 2 clinical factors associated with preoperative postural instability.</p

    ARIX and PHOX2B polymorphisms in patients with congenital superior oblique muscle palsy.

    To identify ARIX gene and PHOX2B gene polymorphisms in patients with congenital superior oblique muscle palsy, 3 exons of the ARIX gene and PHOX2B gene were sequenced by genomic DNA amplification with polymerase chain reaction (PCR) and direct sequencing in 31 patients with congenital superior oblique muscle palsy and in 54 normal individuals. A family with a father and one daughter each having congenital superior oblique muscle palsy was also included in this study. Eleven patients with congenital superior oblique muscle palsy had heterozygous nucleotide changes in the ARIX gene, including 4 patients reported on previously. One patient with atrophy of the superior oblique muscle had a new change of T-4G in the promoter region of the ARIX gene. The other 6 patients had a heterozygous nucleotide change of G153A in the 5'-untranslated region (UTR) of the exon 1 of the ARIX gene. These nucleotide changes of the ARIX gene, taken together, had a significant association with congenital superior oblique muscle palsy(P = 0.0022). One patient and 5 patients had heterozygous nucleotide changes of A1106 C and A1121 C in exon 3 of the PHOX2B gene, respectively, while these changes were absent in the normal individuals. Two patients had both the G153A change in the 5'-UTR of exon 1 of the ARIX gene and the A1121 C change in exon 3 of the PHOX2B gene. In conclusion, the polymorphisms of the ARIX gene and PHOX2B gene may be genetic risk factors for the development of congenital superior oblique muscle palsy.</p

    Sentence Specified Dynamic Video Thumbnail Generation

    With the tremendous growth of videos over the Internet, video thumbnails, providing video content previews, are becoming increasingly crucial to influencing users' online searching experiences. Conventional video thumbnails are generated once purely based on the visual characteristics of videos, and then displayed as requested. Hence, such video thumbnails, without considering the users' searching intentions, cannot provide a meaningful snapshot of the video contents that users concern. In this paper, we define a distinctively new task, namely sentence specified dynamic video thumbnail generation, where the generated thumbnails not only provide a concise preview of the original video contents but also dynamically relate to the users' searching intentions with semantic correspondences to the users' query sentences. To tackle such a challenging task, we propose a novel graph convolved video thumbnail pointer (GTP). Specifically, GTP leverages a sentence specified video graph convolutional network to model both the sentence-video semantic interaction and the internal video relationships incorporated with the sentence information, based on which a temporal conditioned pointer network is then introduced to sequentially generate the sentence specified video thumbnails. Moreover, we annotate a new dataset based on ActivityNet Captions for the proposed new task, which consists of 10,000+ video-sentence pairs with each accompanied by an annotated sentence specified video thumbnail. We demonstrate that our proposed GTP outperforms several baseline methods on the created dataset, and thus believe that our initial results along with the release of the new dataset will inspire further research on sentence specified dynamic video thumbnail generation.     Outcomes of binocular treatment using a Bangerter occlusion film and computer games in patients with intractable unilateral amblyopia

     To report outcomes following binocular treatment using a Bangerter occlusion filter (BF) and computer games in patients with intractable amblyopia.METHODS: Eight patients (4 boys, mean ± SD age: 8.0 ± 0.8 years) with unilateral amblyopia that did not respond to conventional treatments were studied. They were instructed to play action games for one hour a day while wearing spectacles with an adequate level of BF in front of the non-amblyopic eye so that the visual input became the same between the two eyes. They continued this exercise for eight weeks, and we assessed their visual acuity and spatial sensitivity at baseline, and at 4- and 8-week visits. To confirm the maintenance of efficacy after the treatment, we assessed them again at a 12-week visit. RESULTS: The mean log MAR at distance improved from 0.32 to 0.24 at the 4-week visit (p < 0.05), and appeared to continue up to eight weeks, but returned to the baseline level at the 12-week visit (four weeks after terminating the treatment). There was no significant improvement in the mean log MAR at near. Contrast sensitivity significantly improved only at three cycles/ degree (p < 0.05), and this effect persisted until the 12-week visit. The distance log MAR at the 12-week visit had a significant correlation with the strength of suppression for the amblyopic eye at baseline (r = 0.71, p < 0.05). CONCLUSIONS: Binocular treatment improved visual function only in terms of contrast sensitivity at low spatial frequency. Patients who have weak suppression may gain some benefit from this treatment

    Traffic jams without bottlenecks-experimental evidence for the physical mechanism of the formation of a jam

    A traffic jam on a highway is a very familiar phenomenon. From the physical viewpoint, the system of vehicular flow is a non-equilibrium system of interacting particles (vehicles). The collective effect of the many-particle system induces the instability of a free flow state caused by the enhancement of fluctuations, and the transition to a jamming state occurs spontaneously if the average vehicle density exceeds a certain critical value. Thus, a bottleneck is only a trigger and not the essential origin of a traffic jam. In this paper, we present the first experimental evidence that the emergence of a traffic jam is a collective phenomenon like 'dynamical' phase transitions and pattern formation in a non-equilibrium system. We have performed an experiment on a circuit to show the emergence of a jam with no bottleneck. In the initial condition, all the vehicles are moving, homogeneously distributed on the circular road, with the same velocity. The average density of the vehicles is prepared for the onset of the instability. Even a tiny fluctuation grows larger and then the homogeneous movement cannot be maintained.
